What to Bring to Your Green Card Medical Examination:
A part of your immigration medical exam is verifying that your vaccinations are up to date. To avoid multiple visits and unnecessary vaccination costs, please bring:
• Your current and childhood vaccination records, including proof of a COVID-19 vaccination
• A translation of the documents if they’re not in English
• If you need any additional testing or vaccinations, trüHealthNow can provide blood work, X-rays and vaccinations during your visit
Also, bring with you:
• Your completed I-693 form
• Official photo ID, such as a passport, a driver’s license or a state ID
• If you have a USCIS number (A- number), please bring it with you
• Any chest x-rays from within the last year
• If you do not speak English, please bring a translator
